What is a Valid Age Proof for LIC Policy?

There are two types of Age Proof accepted and needs to be submitted for buying a LIC Insurance Policy:
1- Standard Age Proof
2- Non-Standard Age Proof

1. Standard Age Proof

The following certificates or documents are accepted as standard age proof:
• Certified Extract from Municipal or other records made at the time of birth
• School / College Certificate giving Date of Birth (DOB), Date of admission, Admission No, years of study and Classes studied etc.,
• Attested copy of school or College Transfer Certificate (TC), Secondary School Leaving Certificate (SSLC) or Higher Secondary (H.Sc.) mark sheet and SSLC first page
• Certificate of Baptism or certified extract from Family Bible, if it contains age or date of birth
• Identity cards issued by the Defence Departments to defence personnel.
• Identity cards (provided date of birth is mentioned therein) issued by Government, Quasi Government, reputed commercial and industrial undertakings to their employees.
• Marriage certificated issued by Roman Catholic Churches in the case of Roman Catholics.
• Domicile Certificates in which the date of birth stated was proved on the basis of the school or birth certificate and
• Passport
• Horoscope maintained by a Hindu family in a Bahi (or) family Horoscope book can be considered as standard age proof provided Manager (NB) is satisfied with its reliability and originality

When Standard Age Proof is Compulsory?

• For Widows (falling under Female Category III)
• For Minors
• For opting Premium Waiver Benefit under proposals on the lives of minors
• Total Sum Assured in excess of Rs.2 lacs – when Term Rider is opted for
• Total Sum Assured in excess of Rs.2 lacs – when Critical Illness Rider is opted for
• Under Non Medical General (NMG) Professionals and Non-Medical Special (NMS) cases
• Under Non Medical General (NMG) Others – beyond Non-Standard Age proof (NSAPs) limits.

2. Non Standard Age Proof

LIC accepts following Non-Standard Age Proofs (NSAP), if the proposer does not have any of the standard age proof. NSAP are three types:
A- Non-Standard Age Proof – I
B- Non-Standard Age Proof – II
C- Non-Standard Age Proof – III

A- Non-Standard Age Proof – I

• PAN card issued by Income Tax Authority
• Driving license issued by Regional Transport Office (RTO)

B- Non-Standard Age proof – II

• Employee’s State Insurance Corporation (ESIC) card
• Marriage certificates in case of Muslim
• Service record where only year of birth is mentioned

C- Non-Standard Age Proof – III

• Horoscope other than that maintained by a Hindu Family in a Bahi or family horoscope book.
• Election (voter) ID card , if date of issue of the card is available
• Elder’s declaration in LIC Form No 3261
• Self declaration in LIC Form No 5220 (stamped)
• Self declaration in LIC Form No 5096 (unstamped)
• Any other age proof not classified in any of the above (Std, NSAP I & II )

Other Conditions for NSAPs

• LIC Form No 3260 is required for all cases when a Non Standard age proof is submitted.
• When non-standard age proofs are submitted, consistency of ages of father, mother, brother, sister should be verified. If there is any inconsistency in the ages of the family members, standard age proof of the oldest son / daughter should be called for.
