ow to File Death Claim for a LIC Policy?
Step 1: Intimation of Death to LIC
You could report your claim by phone, mail, fax or e-mail as soon as possible. When you call, please have the following information on hand:
1– Name of the deceased.
2– The policy number(s).
3– The date of death.
4– Your relationship with deceased.
5– Your contact number.

Step 2 : Documentation Preparation
There are two types of death claims, early & non early. If the death of insured person takes place within 3 years of starting the LIC Policy, it will be called early death claim otherwise non early death claim and non early death claims are settled within 30 days from the date of receipt of all requirements. Early death claims needs to be investigated first and settled within 6 months from the date of receipt of all requirements.
Non Early Death Claims:
The policies where death occurs after three years from the date of risk or from date of revival/reinstatement, these claims are known as Non Early Death Claims. These policies whould be where premiums are paid up-to-date or where the death occurs within the days of grace. In such cases following documents are required:
1– Letter of death intimation containing date, place, LIC policy no. & reason of death from the claimant/ nominee to the branch manager of the concerned branch.
2– Claimant’s/Nominee’s Proof of relationship with the deceased.
3– Claim form A (LIC Form No.3783) – Claimant’s Statement giving details of the deceased and the claimant. If policy has run for 3 years or more from date or risk, LIC Form No.3783A will be used.
4– Discharge Form (LIC Form No 3801) duly executed by the nominee
5– NEFT Mandate Form (National Electronic Fund Transfer form) for transferring the claim amount in the account of nominee after settlement of the death claim.
6– Certified Extract from Death Register
7– Documentary Proof of Age (if not submitted earlier)
8– Evidence of title to the deceased’s estate if the policy is not nominated, assigned or issued under M.W.P. Act.
9– Original LIC Policy Document

The following additional forms are required if death occurs within three years from the date of risk or from date of revival/reinstatement:
1– Claim Form B (LIC Form No.3784 Revised)– Medical Attendant’s Certificate to be completed by the Medical Attendant of the deceased during his/her last illness.
2– Claim Form B1 (LIC Form No.3816) – if the life assured received treatment in a hospital.
3– Claim Form C (LIC Form No.3785 Revised)– Certificate of Identity and burial or cremation to be completed and signed by a person of known character and responsibility.
4– Claim Form E (LIC Form No.3787 Revised) – Certificate by Employer if the assured was employed person.
5– If death was due to accident or unnatural cause certified copies of the First Information Report (FIR), the Post-mortem report and Police Investigation Report. For most accidental death claims, insurer will also need any accident, autopsy, or toxicology reports.

These additional forms are required to satisfy the genuineness of the claim, i.e., no material information that would have affected our acceptance of proposal has been withheld by the deceased at the time of proposal.
Rider Claims:
The following additional form s are required if there is any rider attached to your LIC policy –
1- For Critical Illness Rider, necessary medical documents such as first investigation report, Doctor’s prescription, Discharge Summery etc are required
2- For Accidental disability rider, Attested copy of FIR, Doctor Certificate of disability, Photograph of the injured with reflecting disablement, Original Medical bills with prescriptions/ treatment papers etc are required.
3- For Hospital cash rider medical documents are required such as Medical & Investigation report, Prescriptions, Medical and Investigation Bills, Discharge Card etc.

Double Accident Benefit:
Following below documents and further procedures are need to be followed.
1- The claim must be lodged with LIC OF INDIA within 120 days from the date of death.
2- Attested copy of F.I.R., Post Mortem report duly attested by the competent court or civil police is required.
3- Spot panchnama, eye witness statement.

Claim will be admitted after acceptance and verification of the above mentioned documents by LIC & clearance by the concerned SDM-LIC.
Step 3: File you Claim with LIC
Please visit LIC Branch (from where the policy had been bought) with your photo id and address proof and submit all the above documents.
As a standard LIC has to settle the claim within 30 days of receipt of all the above documents including further clarification sought by LIC Claims Team. If the claim requires further investigation, LIC has to complete the investigation within six months and pay the claim, from receiving the written intimation of claim.
If you won’t receive the claim with in above timeline, you may file an appeal with LIC or complaint to IRDAIor Insurance Ombudsman.
Things to take care:
1- In the absence of nominations under the policy, the list of all the successors should be submitted on LIC Form No. 3806 and on submission of Declaration Form a claim up to Rs. 250,000 can be settled without succession certificate.
2- In case of policyholder’s unnatural death caused by accident, suicide etc. the LIC may require some additional details besides above mentioned documents depending on the nature of the case.
